Job summary

Qualifications

Role Description

This is a full-time, on-site Hospice Registered Nurse role based in Burlingame, CA. The Hospice RN provides direct nursing care to patients in hospice, including comprehensive assessments, symptom and pain management, medication administration, and coordination of individualized care plans. Day-to-day responsibilities include collaborating with physicians and the interdisciplinary team, educating patients and families on disease processes and comfort-focused care, and documenting all care and communications in accordance with regulatory and organizational standards. The role also involves participating in case conferences, supporting families through emotional and practical aspects of end-of-life care, and ensuring that care is delivered safely, ethically, and in alignment with patient goals and preferences.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of California and current Basic Life Support (BLS) certification.


• Clinical experience in hospice, palliative care, home health, or related acute/chronic care settings, with strong skills in pain and symptom management.


• Demonstrated ability to perform comprehensive patient assessments, develop and implement care plans, and provide evidence-based nursing interventions.


• Strong communication and interpersonal skills, including the ability to educate patients and families, collaborate with diverse care teams, and offer empathetic support.


• Proficiency in electronic health records (EHR) documentation, time management, and adherence to hospice regulations and quality standards.


• Ability to work on-site in Burlingame, CA, reliably manage visits and schedules, and respond effectively in urgent or changing clinical situations.


• Comfort working with end-of-life issues and providing culturally sensitive, non-judgmental care to patients and families from diverse backgrounds.


• Preferred qualifications: Hospice and Palliative Care Certification (CHPN), experience with interdisciplinary case management, and familiarity with Medicare hospice guidelines.
