Responsibilities

• Care Management and Service Delivery tasks including ongoing assessment and nursing care in patients' homes or facilities.
• Prioritize and manage a comprehensive Plan of Care with regular reviews and updates.
• Coordinate with other health care providers and ensure quality hospice care within regulatory standards.
• All other duties as assigned.
