Job summary

Three Oaks Hospice is seeking a compassionate registered nurse to administer skilled nursing care for patients in their homes across Haralson, Polk, Douglas and Bartow counties. You will coordinate care with the interdisciplinary team, patients, families, and referring agencies, and assume responsibility for care coordination.

The role demands strong clinical skills, CPR certification, and the ability to manage complex needs, communicate effectively, and maintain meticulous documentation.

Qualifications

• RN with a nursing degree and current license.
• Hospice or home health experience preferred.
• CPR certification required.
• Proficient with computers and MS Office preferred.

Responsibilities

• Conduct initial and ongoing assessments of patients and family needs.
• Implement individualized plan of care (POC) and revise as needed.
• Educate patient and family about disease, self-care, and end-of-life care.
• Provide training to staff as needed.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical notes showing progress toward goals.
• Coordinate services with the interdisciplinary team (IDT).
• Refer to other services using case management approaches.
• Inform physicians of changes in patient needs and responses.
• Determine scope and frequency of services based on acuity.
• Assess caregiver ability and update plan throughout care.
• Communicate information using current processes and technology.
• Apply admission and re-certification criteria for hospice care.
• Perform on-call responsibilities as assigned.
• Other duties as assigned.
