Job summary

The Pennant Group through Buena Vista Home Health, Palliative Care & Hospice in California seeks an experienced RN to join as a full‑time Hospice Case Manager. You will lead nursing evaluations, manage care plans, and guide families and caregivers to ensure quality end‑of‑life care.

Responsibilities include coordinating services, supervising level staff, and adhering to hospice standards. Valid CA RN license, CPR, and driving eligibility are required, with a preference for a BSN and hospice

Qualifications

Graduate of an accredited school of nursing.
Current RN License within CA.
Bachelor's in Nursing preferred.
Certification in Hospice and Palliative Nursing desired.
A current CPR Certification, Valid Driver’s License and auto insurance.
Minimum of one year experience as a professional nurse within the last 3 years.

Responsibilities

RN Hospice Case Manager: performs comprehensive nursing evaluations and assesses signs of discomfort.
Initiates and manages plan of care; provides nursing follow‑up and caregiver instruction.
Coordinates all services for patients and families to ensure quality of care.
Supervises Home Health Aides, LVNs and LPNs to ensure best patient care.
