Buena Vista Home Health, Palliative Care & Hospice (BV) is seeking an experienced, compassionate Full-time Registered Nurse to join our hospice team in Ventura and Los Angeles Counties, CA. The RN Hospice Case Manager will lead comprehensive nursing evaluations and coordinate services to ensure comfort and quality of life for patients and families.

Requirements include CA RN license, CPR, and driver’s license. BSN preferred and hospice/palliative nursing certification desired.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN License within CA; Bachelors in Nursing preferred.
• Certification in Hospice and Palliative Nursing desired.
• A current CPR Certification, Valid Driver’s License and auto insurance.

Responsibilities

• Performs comprehensive nursing evaluations.
• Assesses for signs and symptoms of discomfort including pain; takes prompt action.
• Initiates and manages plan of care; provides nursing follow-up and caregiver instruction.
• Coordinates all services for patients and families to ensure quality of care.
• Supervises Home Health Aides, LVNs/LPNs, and Homemakers to ensure quality care.
