Job summary

AdvisaCare Hospice in Michigan’s Tri-Cities area is seeking a compassionate Hospice Admission RN to join our team serving surrounding communities. You will assess patients’ needs, coordinate admission processes, and work with an interdisciplinary group to ensure comfort and dignity at end of life.

Qualifications

• Current Michigan RN license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Participate in field visits daily and in-office meetings as indicated.
• Full-time on-call schedule required.

Responsibilities

• Initial and ongoing assessment of patient needs (physical, psychosocial, environmental).
• Implement and revise the plan of care.
• Ensure compliance with hospice policies and Medicare rules.
• Educate patient/family on disease, end-of-life care, and self-care.
• Supervise paraprofessionals providing services to patients.
• Prepare clinical and progress notes; participate in IDG conferences.
• Support performance improvement initiatives.
• Maintain high patient/family satisfaction; communicate effectively.
