Job summary

AdvisaCare is seeking a compassionate RN or LPN to join our Hospice at Home team serving Lakeview, Edmore, Mecosta and nearby areas. You’ll provide one-on-one in-home care focused on comfort, dignity, and emotional support for patients and families.

Responsibilities include initial and ongoing assessments, implementing the plan of care, supervising paraprofessionals, and participating in IDG meetings to coordinate care.

Qualifications

• Current Michigan license in good standing.
• Minimum 1 year hospice experience.
• Ability to participate in field visits daily and attend weekly office meetings.

Responsibilities

• Provide in-home, one-on-one hospice care.
• Perform initial and ongoing patient assessments.
• Develop and update the patient/family plan of care.
• Supervise paraprofessional staff and participate in IDG meetings.
• Ensure compliance with Hospice policies and Medicare requirements.
