Job summary
AdvisaCare is hiring a Hospice RN or LPN to provide compassionate, in-home hospice care across Michigan. You will deliver one-on-one care, support patients and families with dignity, and coordinate with the interdisciplinary team to meet evolving needs.
There are no on-call requirements thanks to our after-hours and weekend team. Full-time or part-time positions available in Lakeview, Edmore and Mecosta areas, with opportunities to grow, comprehensive benefits, and weekly pay.
Qualifications
- Current Michigan license in good standing.
- Minimum 1 year hospice experience.
- Empathetic approach to patients and caregivers.
- Ability to participate in field visits daily and attend weekly meetings.
- On-call availability as required by the role.
Responsibilities
- Provide one-on-one care in patients' homes with compassion.
- Develop and adjust the plan of care with the care team.
- Document progress notes and participate in IDG conferences.
- Support patients and families with dignity and end-of-life care.
- Coordinate with physicians, paraprofessionals, and support staff.
