Job summary

Enhabit Home Health & Hospice in Broken Arrow, OK is seeking a registered nurse to coordinate patient care within the home health setting. You will assess patient and family needs and implement the interdisciplinary care plan, following physician orders and the agreed POC.

Candidates must be a graduate of an approved nursing program, hold an active RN license in Oklahoma, and have at least 1 year of RN experience.

Qualifications

• Must be a graduate of an approved school of nursing.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Medicare home health or hospice experience is preferred.
• Driver license and reliable transportation required.
• CPR certification for healthcare provider preferred.

Responsibilities

• Coordinate care and continuously assess patient and family needs; implement the interdisciplinary care plan.
• Perform services per physician orders and the established plan of care (POC).
• Ensure delivery of care complies with federal and state regulations.
