Qualifications

• Heart to serve patients and families and a passion for providing the best possible care.
• Graduate of an accredited school of nursing with a current state license as a registered nurse.
• 2+ years of nursing experience in a clinical care setting (hospice experience preferred).
• Reliable transportation and ability to lift 80-100 lbs as needed.

Responsibilities

• Evaluate patients and create care plans.
• Communicate with the patient, family, and care team to ensure coordinated care.
• Act as driver of the care team to deliver high-quality, compassionate care.
