Job summary

Enhabit Home Health & Hospice is seeking an experienced RN to coordinate patient care and participate in the interdisciplinary team in a home health setting in Alabama. You will assess patient and family needs, implement the plan of care, and ensure adherence to physician orders and regulatory requirements.

The role requires an active RN license in the state, a minimum of one year of RN experience, and solid basic technology skills, including mobile device use.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Provide coordination of care and assess patient and family needs; implement the interdisciplinary care plan.
• Perform services per physician orders and established plan of care (POC).
• Ensure delivery of care is compliant with federal and state regulations.
