Enhabit Home Health & Hospice in Oklahoma seeks a licensed RN to coordinate patient care in home-based settings, coordinating with an interdisciplinary team to adapt plans of care to evolving needs.

Candidates must be a graduate of an approved nursing program, hold an RN license in the state, have at least one year of RN experience, and possess CPR certification and basic tech skills. A valid driver's license and reliable transportation are required.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed as a Registered Nurse in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Coordinate care and assess patient and family needs; implement the interdisciplinary care plan.
• Ensure services comply with physician orders and the plan of care.
• Ensure delivery of care complies with federal and state regulations.
