Qualifications

• Graduate of an accredited nursing program with current MI RN license.
• Capable of assessing patients and families with compassion and clinical judgment.
• Proficient in using technology and documentation in care coordination.

Responsibilities

• Provide professional nursing care and develop individualized plans of care.
• Perform initial history, assessments, and admissions within 24 hours.
• Educate patients and families about disease, self-care, and end-of-life options.
• Collaborate with physicians, social workers, and spiritual care to support care goals.
• Coordinate durable medical equipment and supplies as needed.
• Document care per standards and regulatory requirements.
• Engage in ongoing professional development and team meetings.
