Job summary

AdvisaCare seeks a compassionate Home Health or Hospice Registered Nurse in Michigan to join a growing team. Flexible part-time and full-time opportunities are available to provide high‑quality, one‑on‑one patient care in the home setting.

You will work in communities such as Traverse City, Kalkaska, Baldwin, Cadillac, Manistee and surrounding areas, delivering care that truly matters and supporting families in challenging times.

Qualifications

• Current Michigan License in good standing.
• Minimum 1 year home health and hospice experience.
• Empathetic approach to patients and caregivers.
• Participate in field visits daily, weekly office meetings as indicated.
• Ability to adapt to patient’s status and needs.

Responsibilities

• Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with AdvisaCare Policies and Procedures, Medicare compliance.
• Consult with and educate patient/family on disease processes, self-care techniques, end-of-life care, ethical concerns.
• Supervision of paraprofessionals providing services to patient.
• Prepare clinical and progress notes, participate in IDG conferences.
• Participate in Home Health and Hospice performance improvement program.
• Maintain high level of customer satisfaction with clients, families and caregivers.
• Communicate information using current process and technology within organization.
