Interim HealthCare Hospice in Eau Claire, WI seeks a compassionate Hospice Registered Nurse (RN) to provide home-based care to patients with six months or less to live. You’ll work within an interdisciplinary team to carry out the plan of care and support families. Qualifications

• Graduate of an accredited nursing program and active Wisconsin RN license.
• Experience in hospice or home health care is preferred.
• Strong communication and compassionate patient care skills.

Responsibilities

• Provide home-based hospice care to patients with six months or less to live.
• Perform patient assessments, monitor vitals, and manage pain and medications.
• Educate families about prognosis, care plans, and hospice goals.
• Collaborate with an interdisciplinary team to execute the plan of care.
