Job summary

Qualifications
- RN with active, unrestricted license in Michigan.
- At least one year of RN experience.
- Community health and Medical-Surgical experience preferred.
- Reliable transportation with valid driver's license and car insurance.
Responsibilities
- Provide home-based care to patients with six months or less to live, focused on quality of life.
- Work with an interdisciplinary team to carry out the patient’s plan of care.
- Perform assessments, check vitals, monitor pain, and administer prescribed medications.
- Ensure accessibility of medical equipment, oxygen, medications and other supplies.
- Educate family on prognosis, care plan changes and hospice goals.
- Take a holistic approach engaging patient, family, mind, body and spirit.
