Job summary

Interim HealthCare of Western Wisconsin is hiring Hospice RNs in Eau Claire, WI. You will provide comfort and support to patients and families during the final chapter of life, in a home-based hospice setting.

This full-time role offers a competitive salary range of $75,000 to $90,000 per year, mileage reimbursement, bonuses, 1:1 nurse-to-patient ratios, and opportunities for CEUs and growth.

Qualifications

• Graduate of an accredited nursing program
• Active RN license in Wisconsin
• Minimum of 1 year nursing experience, hospice/home health preferred
• CPR certification
• Knowledge of state and federal home health regulations
• Strong communication and compassionate care

Responsibilities

• Provide home-based care to patients with six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team to carry out the patient’s plan of care
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medications and treatments
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
• Educate family on prognosis, changes in care plan, medication and hospice goals
• Take a holistic approach to care that engages mind, body, spirit and family

Responsibilities

• Provide home-based care to patients with six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family

Requirements

• Graduate of an accredited nursing program and active RN license in Wisconsin
• Minimum of 1 year of nursing experience, ideally in hospice or home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty, strong communication skills and compassion
