Job summary

Ohio Living is seeking a Registered Nurse (RN) Field Staff to plan, coordinate, and deliver quality home health and hospice care across Ohio. You will assess patient needs, develop care plans, and ensure compliance with state and federal guidelines while partnering with caregivers and the care team.

The role requires Ohio RN license, 1 year of acute care or medical-surgical experience, proficiency with Windows and Microsoft Office, and the ability to travel to patient homes.

Qualifications

• Current unencumbered Ohio RN license to practice.
• One year experience in acute care or medical-surgical nursing.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint) and internet.
• EMR experience preferred.

Responsibilities

• Provide ongoing nursing care to patients in homes and care facilities.
• Develop and implement comprehensive care plans addressing patient needs.
• Communicate changes in condition with the care team and physicians.
• Oversee plan of care and evaluate effectiveness; prepare for IDG meetings.
• Ensure compliance with regulatory standards and quality of care.
