Job summary

The Hospice Registered Nurse (RN) at Cantex provides skilled nursing care to hospice patients in their homes and coordinates with the interdisciplinary team, physicians, patients, and families. The RN performs comprehensive assessments, develops and manages hospice plans of care, and ensures quality patient care and regulatory compliance throughout the hospice care process.

This role requires strong clinical assessment skills, care coordination experience, and the ability to work independently

Qualifications

• Registered Nurse licensed per state requirements.
• Hospice experience preferred.
• HCHB experience preferred.
• Strong clinical assessment, communication, and documentation skills.
• Experience working within an interdisciplinary healthcare team.
• Reliable transportation and valid driver’s license.
• Ability to read, write, and communicate effectively in English.

Responsibilities

• Perform comprehensive nursing assessments for hospice patients.
• Develop and update individualized hospice plans of care.
• Coordinate patient care with the IDT, physicians, patients, and families.
• Educate patients and families on disease process, medications, symptom management, and end-of-life care.
• Maintain timely clinical documentation in the electronic medical record.
• Ensure compliance with Medicare hospice regulations and agency policies.
• Promote compassionate, patient-centered end-of-life care.
