Cantex is seeking a dedicated Hospice Registered Nurse (RN) to provide skilled nursing care in patients’ homes and in our Addison/Dallas area coverage. You will assess, plan, implement, and coordinate care with the IDT, physicians, patients, and families to ensure high-quality hospice services and regulatory compliance.

The role requires strong clinical assessment, care coordination, and the ability to work independently in a hospice setting while collaborating with the interdisciplinary care

Competenze

• Active RN license valid in Texas.
• Hospice experience preferred.
• HCHB experience preferred.
• Strong clinical assessment, communication, and documentation skills.
• Experience coordinating care with an interdisciplinary team.
• Knowledge of hospice regulations and care planning.
• Reliable transportation and valid driver’s license.

Mansioni

• Perform initial and ongoing comprehensive hospice nursing assessments.
• Develop and update individualized hospice plans of care.
• Coordinate patient care with the Interdisciplinary Team (IDT), physicians, patients, and families.
• Educate patients and families on disease process, medications, symptom management, and end-of-life care.
• Determine scope and frequency of nursing services based on patient condition and care needs.
• Supervise and provide clinical direction to Hospice Aides and LVN/LPN staff.
• Maintain accurate and timely clinical documentation in the electronic medical record.
• Communicate changes in patient condition to physicians and interdisciplinary team members.
• Ensure compliance with Medicare hospice regulations, documentation standards, and agency policies.
• Participate in case conferences, IDT meetings, and care coordination activities.
• Monitor assigned cases to ensure compliance with third-party payer and hospice requirements.
• Promote hospice philosophy and provide compassionate, patient-centered end-of-life care.
