Job summary

Cantex is seeking an experienced Hospice Registered Nurse (RN) to provide skilled nursing care to patients in their homes within Grayson County/Denison/Gainesville territory. The RN will coordinate care with the IDT, physicians, patients, and families, and develop comprehensive plans of care to ensure high-quality, compliant hospice services.

Qualifications

• RN license required per state regulations.
• Hospice experience preferred.
• HCHB experience preferred.
• Strong clinical assessment, communication, and documentation skills.
• Experience working with an interdisciplinary healthcare team.
• Knowledge of hospice regulations, documentation, and care planning.

Responsibilities

• Perform initial and ongoing comprehensive nursing assessments for hospice patients.
• Develop and update individualized hospice plans of care for quality and continuity.
• Coordinate patient care with the Interdisciplinary Team (IDT), physicians, patients, and families.
• Educate patients and families on disease process, medications, and symptom management.
• Determine scope and frequency of nursing services based on patient condition.
• Supervise and provide direction to Hospice Aides and LVN/LPN staff.
• Maintain accurate clinical documentation in the EMR.
• Communicate changes in patient condition to physicians and IDT members.
• Ensure compliance with Medicare hospice regulations and agency policies.
• Participate in case conferences and care coordination activities.
• Monitor assigned cases for third‑party payer and hospice requirements.
• Promote hospice philosophy and compassionate, patient‑centered end‑of‑life care.
