Job summary

JOL Healthcare, Inc. is seeking an experienced registered nurse to coordinate hospice care and expertly triage incoming clinical calls. You will work to ensure timely communication with clinicians and families, and support daily HomeCare HomeBase workflows.

Responsibilities include assisting with IDT meetings, scheduling, and documentation, while promoting CMS compliance and high-quality patient-centered care. A strong clinical assessor with care coordination skills is essential.

Qualifications

• Active RN license in the state of practice or compact state.
• BSN preferred by organization.
• Minimum 2–3 years nursing experience; hospice/home health or case management preferred.

Responsibilities

• Triage and promptly respond to incoming clinical calls during assigned hours.
• Escalate urgent patient concerns to the appropriate clinician.
• Support IDT meeting prep and ensure care plans are current and documented.
• Assist with admissions, transfers, discharges, and other clinical coordination activities.
• Maintain regulatory compliance and CMS participation with timely documentation.
