Job summary

Holisticare Hospice is seeking a Per Diem Registered Nurse Case Manager to serve patients in Atlantic and Cape May Counties, NJ. The role focuses on assessment, planning, and coordination of hospice services with a compassionate, patient-centered approach.

The RN will collaborate with an interdisciplinary team, educate families, and provide direct nursing care to ensure comfort and quality of life. Evening shifts approximately three times a week.

Qualifications

• Hold current unencumbered RN license.
• Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
• Spanish speaking preferred; CPR certification may be required by state.

Responsibilities

• Respond to clinical referral information with care and expertise for patients and families.
• Collaborate with Clinical Director to assess eligibility and plan appropriate hospice services.
• Coordinate direct patient care and provide comfort and support.
• Deliver nursing care rooted in established principles and techniques to ease pain and improve quality of life.
• Educate patients and families to participate in personalized care plans.
