Job summary
Hospice of the West is seeking a Per Diem Hospice RN Case Manager to join our compassionate nursing team. You will deliver hands-on hospice care, coordinate services, and guide patients and families through every step of their hospice journey in Cottonwood and Yavapai County.
As an RN, you’ll collaborate with our interdisciplinary team, educate families, respond to referrals, and provide comfort focused on dignity and quality of life.
Qualifications
- Current unencumbered RN license.
