Job summary

UnitedHealth Group is seeking a Per Diem Registered Nurse Case Manager for hospice services, delivering the plan of care and ensuring high-quality patient outcomes in home settings.

The role requires an unrestricted RN license, with 1+ year of clinical experience and a valid driver's license. CPR is preferred or obtainable within 90 days, and benefits and competitive hourly pay are provided.

Qualifications

• Current unrestricted RN licensure in state of practice.
• 1+ years of clinical home health or hospice experience preferred.
• CPR certification preferred or ability to obtain within 90 days of hire.

Responsibilities

• Perform initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
• Assess patient/family needs and re-assess at least every 15 days.
• Initiate and coordinate plan of care with accurate clinical notes.
• Document problems, goals, interventions, and patient/family response.
• Collaborate with patient/family, attending physician and IDG members to deliver daily care.
