Job summary

Sanctuary Hospice is seeking a full-time Hospice RN Case Manager (RNCM) to join our nursing team providing exceptional end-of-life care in Rockford, IL and surrounding areas. Schedule Monday–Friday, 8:00 am–4:30 pm, with territory coverage.

You will deliver hands-on hospice care, coordinate services, educate patients and families, and collaborate with the Clinical Director to ensure compassionate, evidence-based care.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct, compassionate nursing care and coordinate services to ensure patient comfort.
• Deliver nursing with evidence-based practices to ease pain and improve quality of life.
• Educate and empower patients and families to participate in care plans.
• Share in call duties as part of the interdisciplinary team.
