Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Champaign, IL. We offer a Monday–Friday schedule, 8:00 AM–4:30 PM, with on-call opportunities for additional compensation.

As a Hospice RN Case Manager you will coordinate patient care, manage plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life for patients at the end of life.

Qualifications

• Active RN License required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards and plans of care.
• Complete scheduled visits, admissions, and clinical interventions with documentation in HCHB.
• Respond to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisziplinar team to coordinate care and support outcomes.
• Educate patients and families on symptom management and end-of-life care while offering compassionate support.
