Job summary

Ennoble Care seeks a Full-time Hospice Registered Nurse Case Manager to serve GA communities including Rockdale, DeKalb and Newton counties. You will join an interdisciplinary team to assess, plan, and coordinate comfort-focused care for patients and support for families.

As a Georgia-licensed RN, you will document in the EMR, communicate with physicians, and guide caregivers. Strong assessment, organization, and compassionate end-of-life care are essential for success.

Qualifications

• Must be a licensed RN in Georgia.
• Hospice experience preferred.
• Current CPR certification.
• Strong assessment and problem-solving skills.
• Excellent written and verbal communication.

Responsibilities

• Manage a patient caseload with nursing assessment, planning, and care.
• Coordinate care with physicians, Hospice staff, and agencies.
• Document in electronic medical records accurately and timely.
• Participate in Interdisciplinary Group meetings.
• Educate caregivers and supervise NAC every two weeks.
• Support bereavement and death processes.
