Job summary

Ennoble Care is seeking a Full-time Hospice Registered Nurse Case Manager to serve Gwinnett, GA and surrounding areas. You will work with an interdisciplinary team to assess, plan, and provide compassionate hospice care in diverse home settings.

The role requires GA RN licensure, 1–2 years of nursing experience (hospice preferred), excellent communication, and ability to coordinate with physicians and caregivers. Travel with a reliable vehicle is expected.

Qualifications

Active RN license in Georgia.
1–2 years of professional nursing experience; hospice preferred.
Ability to assess needs and respond to patients and caregivers.
Skilled in nursing practice; handle emotional stress; adapt to different lifestyles.
Strong observation and reporting skills; produce accurate care documentation.
Team player with interdisciplinary care approach.
Valid driver’s license and reliable vehicle for year-round travel.
Able to take direction and work independently within a team.

Responsibilities

Manage a patient caseload assigned by the Patient Care Coordinator.
Lead assessment, planning, implementation, and evaluation phases of care.
Coordinate with attending physicians and hospice staff for optimal patient care.
Maintain regular communication with physicians regarding patient plans.
Seek input from team members to support the care plan.
Document accurately in the electronic medical record system.
