Job summary

Ennoble Care seeks a Full-time Hospice Registered Nurse Case Manager to serve Gwinnett, GA and surrounding areas. The RN will perform skilled nursing assessments, care planning, and coordination under the Director of Nursing and the interdisciplinary team.

Responsibilities include caseload management, collaboration with physicians and Hospice staff, and thorough documentation in EMR. Requires GA RN license and hospice experience; driver’s license required.

Qualifications

• Active Georgia RN license with no encumbrances.
• Preference for hospice or palliative care experience.
• Able to work within an interdisciplinary team.

Responsibilities

• Manage a patient caseload with planning and evaluation.
• Coordinate care with physicians, Hospice staff, and agencies.
• Document accurately in electronic medical records.
