Job summary

BAYADA Home Health Care is seeking a Hospice RN Case Manager in Spring House, Pennsylvania. The role involves delivering compassionate palliative care at patients' homes within Montgomery and Bucks counties, conducting regular visits, and coordinating with a multidisciplinary team.

Eligibility includes PA RN license and preferred hospice experience, with travel to assigned cases and potential on-call duties.

Qualifications

• Active and current license to practice as a professional RN in PA.
• One year of hospice experience strongly preferred.
• Previous home healthcare experience welcomed, but not required.
• Ability to travel to cases as assigned and provide compassionate care.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medications, perform wound care, treatments and procedures, and provide comfort care.
• Monitor clients' conditions and report changes to supervisor.
• Follow up with and document doctors' orders accurately.
• Occasional on-call after hours and/or weekends.
