BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager, Hospice for a nursing job in Fairless Hills, Pennsylvania. You will be part of a multidisciplinary team delivering high-quality palliative care at home while managing cases in the Bucks County area.

Qualifications

• Active and current PA RN license.
• One year hospice experience preferred.
• Ability to travel to cases.
• Strong written and verbal communication.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments and comfort care.
• Monitor clients’ conditions and report changes to the manager.
• Follow doctors’ orders and document actions accurately.
• On-call after hours and/or weekends as needed.
