Job summary

ConnectedHealth seeks a dedicated Hospice RN Case Manager in Boise, Idaho to provide compassionate nursing care at patients' homes. You will coordinate interdisciplinary hospice teams, assess needs, and educate families while ensuring compliance and documentation.

Ideal candidates hold an Idaho RN license (unrestricted), ADN with BSN preferred, and BLS/CPR certification. Hospice or palliative care experience is a plus, with strong organizational and communication skills essential.

Qualifications

• Idaho RN license (unrestricted) or eligibility for Idaho licensure.
• BLS/CPR certification for healthcare providers.
• Valid driver's license and reliable transportation.
• Strong clinical assessment, documentation, communication, and organizational skills.

Responsibilities

• Provide compassionate, skilled nursing care to hospice patients in homes and other settings.
• Serve as primary case manager for assigned patient caseload.
• Conduct comprehensive nursing assessments of physical, emotional, psychosocial, and spiritual needs.
• Develop, implement, and regularly update individualized hospice plans of care.
• Coordinate care with physicians, aides, social workers, chaplains, volunteers, and other team members.
• Monitor patient condition changes and adjust plans accordingly; communicate significant changes to physicians and medical director.
• Educate and provide emotional support to patients and families on disease progression, symptom management, medications, and end-of-life care.
• Manage pain and other symptoms, administer medications per orders, and evaluate intervention effectiveness.
• Assist families in understanding hospice services and comfort-focused care goals.
• Coordinate care transitions and maintain accurate, timely clinical documentation.
• Participate in interdisciplinary meetings, collaborate with team to ensure continuity and quality of care.
• Respond to urgent patient and family needs within hospice scope; maintain confidentiality and adhere to regulations.
