Job summary

Elite Home Health and Hospice in Idaho is seeking a compassionate Registered Nurse to join our home health and hospice team. The RN will plan, organize, and direct patient care in patients' homes, using a patient-centered approach to improve quality of life and outcomes.

Responsibilities include assessments, care planning, medication administration, teaching patients and families, coordinating with an interdisciplinary team, and participating in on-call duties.

Qualifications

• Graduate of NLN-accredited school of nursing.
• Minimum of two years of experience in home health, hospice, or public health preferred.
• Registered nurse with current Idaho license to practice nursing.

Responsibilities

• Performs initial and ongoing assessments of patient and family to determine home health or hospice needs.
• Develops and implements care plans based on nursing diagnoses.
• Administers medications and treatments as prescribed; monitors and documents outcomes.
• Coordinates with physicians and interdisciplinary team; revises plan of care as needed.
• Provides patient and family education on health management and self-care.
• Participates in on-call duties and ensures equipment and supplies are available.
