Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking an RN Case Manager to join our Dalton, GA hospice team. You will assess patients’ needs, develop care plans, administer medications, and support families in a team-based, patient-centered environment.

This full-time role emphasizes clinical excellence, on-call rotation, documentation, and collaboration with an interdisciplinary team to ensure dignity and comfort at the end of life.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
