Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Registered Nurse Case Manager to provide hospice care in McDonough, GA and surrounding counties. You will assess patients, develop care plans, administer treatments, manage pain, educate families, and coordinate with an interdisciplinary team in a home-based setting.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in the state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a vehicle in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient’s wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
