Job summary

Qualifications

• Current and valid RN license in the state of practice.
• Minimum of 2 years of nursing experience, preferably in acute care, home health, or hospice.
• Knowledge of end‑of‑life care and symptom management.

Responsibilities

• Assesses patients’ medical needs and develops individualized care plans.
• Ensures appropriate symptom management and medications.
• Serves as the primary clinical liaison between patients, families, and the hospice team.

Skills

Strong assessment and clinical decision‑making skills

Excellent communication and interpersonal skills

Ability to work independently

Basic computer proficiency

Education

Current and valid Registered Nurse (RN) license

Minimum of 2 years of nursing experience
