Midland Care Connection in McPherson, KS, seeks an experienced RN Case Manager for its Community Health hospice program. The role coordinates end-of-life care, develops care plans, manages symptoms, medications, and therapies, and acts as the primary clinical liaison among patients, families, and the interdisciplinary hospice team.

Competenze

• Current and valid RN license in the state of practice.
• Minimum of 2 years of nursing experience, preferably in acute care, home health, or hospice.
• Strong assessment and clinical decision-making skills.
• Knowledge of end-of-life care, symptom management, and palliative care principles.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage a caseload effectively.
• Basic computer proficiency for electronic medical records and documentation.
