Job summary

Midland Care Connection in Topeka, Kansas is seeking an experienced RN Case Manager to coordinate comprehensive hospice end-of-life care. You will assess patients, develop individualized care plans, and ensure symptom management while maintaining comfort and dignity at the home.

The role requires strong communication with families and an interdisciplinary team, the ability to manage a caseload, and knowledge of palliative care principles.

Qualifications

• RN license in state of practice required.
• Minimum of 2 years nursing experience, preferably in hospice, home health, or acute care.
• Strong assessment and clinical decision-making skills.
• Excellent communication and interpersonal skills for interacting with patients, families, and interdisciplinary teams.
• Ability to work independently and manage a caseload.

Responsibilities
