Job summary

Compassus is seeking a Hospice Registered Nurse / RN Case Manager to coordinate in-home hospice care. In this role you will build trusting relationships with patients and families, support them through care planning, and collaborate with the care team to ensure comfort and dignity in the home setting.

Required RN licensure, minimum two years of RN experience (hospice preferred), CPR/BCLS, and a valid driver’s license.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse; hospice experience preferred.
• Current CPR and BCLS certification.
• Current driver’s license.

Responsibilities
