Job summary

Affinity Health Management seeks a dedicated registered nurse to plan, organize and direct hospice care within patients' homes and communities. The RN will build on community resources to coordinate services for individuals and families, emphasizing education and supportive care.

You will assess, plan and implement patient care, collaborate with the interdisciplinary team, and provide leadership in care coordination while ensuring safety and quality in a home-based hospice setting.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in good standing.
• One year of nursing experience within last three years; BSN with two years in public health/home hospice preferred.
• Excellent observation, communication, problem-solving and math skills.
• Must have CPR certification and a valid driver’s license.

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient and family involvement.
• Administer medications and treatments per physician's orders.
• Counsel patient and family in meeting nursing and related needs.
• Maintain accurate clinical notes and communicate with the physician as required.
• Participate in on-call duties and ensure equipment and supplies are available.
• Supervise hospice aides when required.
• Engage in quality assessment and performance improvement activities.
