Job summary

Affinity Health Management seeks a Registered Nurse experienced in hospice care to lead in-home patient care across Southern Allegheny County. The RN will assess, plan, implement, and evaluate care, collaborating with families and the interdisciplinary team to ensure comfort and quality of life.

The role includes on-call duties and ongoing professional development. Responsibilities emphasize comprehensive assessment, care planning, patient and family education, and coordination with physicians

Qualifications

• Graduate of an accredited school of nursing.
• Must hold RN license in good standing.
• Experience in home health, hospice, or public health preferred.
• Excellent observation, verbal and written communication skills.
• Ability to lift and transfer patients and work with ambulatory care.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments.
• Provide professional nursing care using the nursing process.
• Develop and revise plans of care based on patient status and needs.
• Administer medications and treatments as prescribed.
• Counsel patients and families on nursing needs and self-care.
• Coordinate patient care as Case Manager with clinical team.
• Maintain accurate clinical notes and document pain/symptom changes.
• Communicate with physicians and community health professionals on care plans.
• Participate in on-call duties and ensure equipment and services are available.
• Support quality improvement activities and continuing education.
