Job summary

Gentiva Hospice in Tennessee is seeking a dedicated Hospice RN / RN Case Manager to guide patients and families through the hospice journey, delivering skilled nursing care in the home or wherever they call home.

You will perform comprehensive assessments, develop and update plans of care, coordinate with an interdisciplinary team, educate patients and caregivers, and ensure timely documentation while complying with regulatory standards. On-call responsibilities may apply.

Qualifications

• RN license in the state of employment.
• CPR certification.
• Current automobile insurance and valid driver’s license.
• Hospice or palliative care experience preferred.
• Strong assessment and communication skills.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams including physicians.
• Educate patients, families and caregivers on disease, care and end-of-life options.
• Document timely clinical records and maintain compliance with regulations.
• Participate in on-call and patient support activities.
