Qualifications

• CA RN License is mandatory.
• Compassionate and caring attitude is essential.
• Experience with HomecareHomebase (Pointcare) is preferred.
• BLS/CPR certification is required.
• Must possess a valid California Driver License.

Responsibilities

• Plans, organizes, and directs hospice services for patients.
• Completes assessments and maintains clinical notes.
• Develops care plans to meet patient needs.
• Assists patients with daily living activities.
• Acts as Case Manager and coordinates care.
