Job summary

Acacia Health is seeking a compassionate RN in Palm Desert, CA, to direct hospice care for patients and families. In this role, you will be responsible for assessing patient needs, developing care plans, and providing support for daily living activities.

The ideal candidate will have a California RN License and at least one year of experience in hospice nursing. A commitment to a patient-first philosophy and effective communication with healthcare teams is also essential.

Qualifications

CA RN License is mandatory.
Compassionate and caring attitude is essential.
Experience with HomecareHomebase (Pointcare) is preferred.
BLS/CPR certification is required.
Must possess a valid California Driver License.

Job Description Summary

The RN plans, organizes and directs hospice, and plans and directs services to meet the needs of patients and their families within their homes and communities.

Essential Job Functions/Responsibilities

Completes an initial, comprehensive and ongoing assessment of patients.
Initiates the plan of care and makes necessary revisions as needs change.
Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self‑sufficiency and optional comfort care.
Acts as Case Manager and assumes responsibility to coordinate patient care.
Completes, maintains and submits accurate and relevant clinical notes in HomecareHomebase (Pointcare).
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
Teaches the patient and family/caregiver self‑care techniques as appropriate.
Attends interdisciplinary group meetings.
Fulfills the obligation of requested and/or accepted case assignments.
