Job summary

Aura One Hospice in Palm Desert, CA is seeking a Registered Nurse to provide compassionate end-of-life care to patients and families in home, facility, or inpatient settings. Qualifications

• Active California RN license in good standing.
• Minimum 1 year of nursing experience; hospice/home health preferred.
• CPR/BLS certification.
• Valid driver's license and reliable transportation for patient visits.
• Proficiency with EMR systems.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans for hospice patients.
• Administer medications, treatments, and pain management per physician orders.
• Provide skilled nursing visits to patients in home, facility, or inpatient settings.
• Educate and support patients and families on disease progression and end-of-life planning.
• Collaborate with interdisciplinary team including physicians, social workers, chaplains, and aides.
• Document patient care accurately using EMR.
• Respond to after-hours on-call needs and urgent patient concerns.
