Job summary

The Elizabeth Hospice is seeking an RN Case Manager to provide professional nursing care for adults, pediatric, and perinatal patients across our service area. You will assess, plan, implement, and evaluate care with the IDT, while ensuring coordinated communication with families and internal/external teams.

The role requires field visits in North San Diego and South Riverside counties, travel, and on-call coverage.

Qualifications

• Associate's degree in nursing or BSN required.
• CA RN license (active) required.
• Current CA driver’s license with proof of auto insurance.
• CPR certification and negative TB skin test.
• Hospice experience preferred.

Responsibilities

• Develops written plan of care with the IDT.
• Oversees implementation and updates plan to meet needs.
• Directs care by LVN and HA per policy.
• Supervises LVN/HA per TEH requirements.
• Communicates changes to IDT and primary MD.
• Maintains patient documentation in electronic charting.
• Ensures compliance with Policies, Standards, and regulatory rules.
• Provides caregiver education to patient and family.
• Participates in 24/7 on-call coverage and census management.
• Performs admission assessments when needed.
