Heartland Hospice in the United States seeks a compassionate RN Case Manager to lead end‑of‑life care for patients in their homes. You will assess, plan, and deliver skilled nursing care, coordinating with an interdisciplinary team to ensure comfort and dignity.

The role emphasizes education for families, coordination of services, and meticulous documentation, with on‑call duties and a focus on compassionate communication across diverse patient populations.

Qualifications

• RN with at least 1 year in hospice, home health, or related care.
• Excellent patient assessment and case management skills.
• Strong communication with patients, families, and IDG.
• Knowledge of hospice regulations and standards.

Responsibilities

• Perform comprehensive patient assessments and reassessments in person or via telecommunication.
• Deliver nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans and visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, chaplains, and other team members.
• Educate patients and caregivers on disease, self-care, end-of-life care, medications, nutrition and dietary needs.
• Document timely clinical records and maintain infection control and compliance standards.
• Participate in on-call and patient support activities as applicable.
