Job summary

Heartland Hospice is seeking a dedicated Hospice RN / RN Case Manager to guide patients and families through the hospice journey in home and community settings. You will assess, educate, and coordinate with an interdisciplinary team to deliver compassionate end-of-life care.

Requirements include an active US RN license and at least 1 year of RN experience, with hospice or home health preferred. A supportive team, comprehensive benefits, and an emphasis on patient- and family-centered care are

Qualifications

Active US RN license in Michigan (state-specific requirements apply).
Minimum 1 year RN experience in acute care, hospice or home health.
Hospice or palliative care experience preferred.

Responsibilities

Perform comprehensive patient assessments and reassessments in home or clinic settings.
Deliver nursing interventions and symptom management aligned with goals of care.
Develop and update individualized plans of care with visit frequency.
Coordinate care across IDG and physicians for optimal outcomes.
Educate patients and caregivers about disease, end-of-life care, and medications.
Communicate condition changes to the care team and ensure timely interventions.
Document records promptly and maintain infection control standards.
Support ongoing quality improvement and scheduling initiatives.
