Job summary

Angels Care Hospice in Glenwood, IA is seeking a dedicated Registered Nurse Case Manager to coordinate all aspects of the patient's home care. You will collaborate with medical teams and perform admission assessments.

The role requires an RN license, CPR, a valid driver's license and reliable transportation; preferred one year of RN experience. We offer strong benefits and opportunities for professional growth.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• The Registered Nurse Case Manager coordinates all aspects of the patient's home care.
• Interact with other medical disciplines as needed and provide professional care to the patient.
• Perform the admission visit to the patient in the home, determine eligibility for home care services, and develop the plan of care to be followed.

Education

Nursing degree (Diploma/Associate/Baccalaureate)
